We are currently seeking a Reviewing and Assessment officer to join our Adults team in Surrey. As the successful applicant you will need: To manage a caseload which may include the more vulnerable users or those with a variety of problems; such responsibility to include taking action in relation to liberty or safety of users in emergency situations. To make appropriate assessments of individuals social care needs in collaboration with health and other care agencies taking into account the wishes of individuals and their carers. To plan relevant response to meet individual needs in co-operating with relatives, carers, friends, health and other care agencies. To secure the delivery within the budget of care plans involving contracts with service suppliers. To maintain an effective system for co-ordinating, monitoring and reviewing care plans, and service standards; to identify gaps in services and advise managers accordingly. To maintain a range of costed resource information accessible to users, carers and other colleagues and which can be deployed to meet individual needs
